I confirm that I am over 16 and that I am happy to work as a volunteer for Able2B. I understand I will be under the supervision of staff at Able2B and will follow their policies and procedures at all times.

I confirm that I have read and understood the Policies and Procedure Documents of Able2B (available by request or on the website) and agree to the terms and conditions of the documents.

I confirm that I do not hold any criminal convictions or have been barred from working alongside children or vulnerable adults.

I am happy to provide proof of identification in the form of a passport, student photo card, driving license or equivalent.

Covid 19
I confirm that I currently do not have any symptoms relating to Covid-19 and that I will be sure to assess I am fit and well before attending the gym sessions each week and not attend if I am unwell or any direct family member is unwell or has Covid-19. I confirm I will maintain social distancing at all times whilst at the gym and adhere to the Covid-19 policy that I have read and understand. I recognize that my attendance at the gym is at my own risk with regards to Covid-19 transmission.

DBS Check
I confirm I already have a DBS certificate and can provide a copy to Able2B.  I confirm I am happy for Able2B to carry out a DBS check if they feel this is appropriate before I begin volunteering

Volunteering
II understand that I undertake volunteering knowing there are risks when under-taking fitness training type sessions/exercises.
